This website is intended as a tool to allow you to track your progress while you are preparing for or have had weight loss surgery with the REALIZE® adjustable gastric band or a gastric bypass or sleeve gastrectomy using an Ethicon Endo-Surgery, Inc. device. In order to utilize the specific features of this website, you will be required to register and log on as a user. Once you are logged on, you will be able to personalize your profile by entering specific information to track your progress such as the date of your surgery or appointments, weight, food diary and band adjustments. The website also allows you to keep track of your goals, motivations and challenges. This information is stored in our database and used and disclosed in accordance with this policy. Except where expressly stated, providing any information on the website is optional.
Physician access: Your physician (as well as specific staff at your physician’s office authorized by your physician) will be able to view your personal information, including site usage data, weight entries, and food diary entries between scheduled visits. Access to this data is intended to assist your physician in monitoring your progress. Some of the information you provide, such as date of surgery or appointments, may be used to catalogue your profile to facilitate access by your physician.
If you change the physician associated with your care, you will need to update the Physician's Practice in your Patient Information in “My Profile”.
E-mail reminders and promotional materials: From time to time, you may receive e-mail reminders sent by us to keep you on track with your weight loss goals. You may also choose to stay informed by e-mail about the operation of our service, and to receive promotional or other announcements that Ethicon Endo-Surgery, Inc. or its affiliates may send out from time to time to the e-mail address [or mailing address] that you provide during registration.
You may change your settings at any time in your Patient Information in “My Profile” once you have logged in to the site.
We may disclose personally identifiable information you provide via this site to third parties that are not Johnson & Johnson affiliates, but only:
- to respond to law enforcement requests or where required by applicable laws, court orders, or government regulations.
Some of the personal information we collect may be stored or processed at computers located in other jurisdictions, such as the United States, whose data protection laws may differ from the jurisdiction in which you live.
