This website is intended as a tool to allow you to track the progress of your patients who have received a REALIZE® adjustable gastric band or a gastric bypass or sleeve gastrectomy using an Ethicon Endo-Surgery, Inc. device.
In order to access this web site, you would have provided a Physician’s Practice ID number. As part of that process, you would have agreed to various terms designed to protect the privacy of your patients. Please familiarize yourself with those terms.
In order to access this website, you will be required to register and log on as a user. You will be required to provide specific identifying information, including the name and address of your practice. This information is stored in our database and disclosed in accordance with this policy. Except where expressly stated, providing any information on the website is optional. Once you are logged on, you will be able to personalize your profile by identifying authorized users and the information they may access.
Promotional material: You may choose to stay informed by e-mail about the operation of our service, and to receive promotional or other announcements that Ethicon Endo-Surgery, Inc. or its affiliates may send out from time to time to the e-mail address that you provide during registration.
